Trauma del porta-hepatis
Abstract
Se revisaron las historias de quienes ingresaron al Hospital Universitario del Valle desde enero de 1987 a diciembre de 1991 con trauma abierto o cerrado a las estructuras del porta-hepatis (arteria hepática, vena porta, colédoco y/o conductos hepáticos). Hubo 23 pacientes con edad promedio de 25.1 años. En 4 el trauma fue cerrado y en 19 abierto. La estructura más comprometida fue la vena porta en 16 casos; la arteria hepática en 7; el colédoco en 6; y los conductos hepáticos en 2; 10 (40%) pacientes ingresaron al hospital con TA 0/0 y 1 en asistolia. El daño asociado más frecuente fue el del hígado en 20 (82%), seguido de páncreas en 6 (26%). El daño vascular asociado más común fue el de la vena cava en 8 enfermos. Se midió el índice de trauma abdominal (ITA) que en todos era mayor de 20. La mortalidad fue de 18 (78%) y en 11 (48%) fue intraoperatoria.Authors
